What happens in an eye examination, in order
Every step of the forty-five minutes, what each one is for, and which of them is the reason the appointment exists. Including the puff of air, and why it is not the important part.
Forty-five minutes, in this order, for a comprehensive examination. The step people talk about afterwards is nearly always the puff of air, and it is the smallest part of what the visit is actually for — the two steps either side of it, looking at the front and the back of the eye directly, are where a health finding gets made.
History
What you have noticed, what you do all day, and what medicines you take. It is the part that takes the longest on a first visit, and it decides which of the tests that follow actually matter for you.
Distance and near vision
Each eye measured separately, then both together — how well you see a chart across the room and a page in your hand, before anything is done to change it.
The prescription, refined
Lens after lens, until two choices look the same rather than until one looks sharp. That is deliberate: the endpoint of a refraction is the point where you can no longer tell the difference, not the point where the letters look their best.
How the eyes move, point and focus as a pair
Two eyes that see well on their own can still be working badly together, and this is the part of the visit most people have never had explained to them.
The pressure inside each eye
The puff of air, or a small probe that touches the surface instead. This is the step people remember and dread, and it is one number among several — pressure, the visual field, and the appearance of the optic nerve are worth far more together than any one of them read alone, so a single reading is not the part of the visit that actually decides anything.
The front of the eye, under magnification
The lids, the surface and the front chamber, looked at through a microscope. Most of what causes discomfort day to day lives here.
The back of the eye
The retina and the optic nerve, looked at directly or photographed. This and the step before it are where a health finding is actually made — the two quiet steps in the middle of the appointment, not the one with the puff of air.
A written prescription
Given to you unasked, whether or not you buy anything here. It is yours, and it works anywhere.
Why the order is the order
History first, because it decides what the rest of the appointment should pay attention to. Sight measured before health, because a prescription is the part most people came in expecting and the quickest to finish. The two health steps come last, and they are not an afterthought for being last — they are the steps that take the longest to do properly and the ones nothing about how you see would have told you were needed.
See it for yourself
Forty-five minutes, in that order, with a written prescription at the end of it.
